PCMH

What is a Patient Centered Medical Home?

A Patient-Centered Medical Home (PCMH) is a trusted partnership between a patient and a doctor-led healthcare team. It focuses on providing coordinated, whole-person care that is centered around the patient’s needs, goals, and overall health.

PCMH is built on a shared commitment between the patient and the healthcare team, where both work together to support better health outcomes.

What can Patient Centered Medical Home do for you?

  • A PCMH can help by:

    • Providing more personalized care because your healthcare team knows you and your health history.
    • Helping guide you through the healthcare system to ensure you receive the care you need.
    • Coordinating care between your primary care provider, specialists, hospitals, pharmacies, and other healthcare services.
    • Improving access to healthcare services and communication with your care team.
    • Supporting preventive care and healthy lifestyle choices.
